TENANT SCREENING DISCLOSURE Before submitting your application, please read the following disclosure carefully. 1. FEES AND DEPOSITS Any application fee, screening fee, holding deposit, or security deposit charged in connection with this application will be disclosed to you in writing by the landlord before you are charged. There is no application fee for this application unless the landlord has expressly stated one. 2. INFORMATION ACCESSED DURING SCREENING The following information may be accessed as part of the screening process: • Credit report and credit history • Criminal records search • Sex offender registry • Eviction, judgment, and lien records • Previous landlord references • Income and employment verification 3. SCREENING STANDARDS The same screening criteria are applied to every applicant. Criminal history, if considered, is reviewed only after a conditional offer of housing and is limited to recent records. Eviction records are considered only where a judgment was entered, and sealed records are never considered. Credit reports are evaluated in full — a credit score alone is not the sole basis for a decision. 4. YOUR RIGHTS UNDER THE FAIR CREDIT REPORTING ACT (FCRA) These rights are provided by the federal Fair Credit Reporting Act (15 U.S.C. § 1681 et seq.): • You have the right to dispute inaccurate or incomplete information directly with the consumer reporting agency, which must reinvestigate at no cost to you. • If your application is denied or otherwise adversely affected based on information in a consumer report, you will receive an adverse action notice identifying the consumer reporting agency, and you may obtain a free copy of your report from that agency within 60 days. • The name and contact information of the consumer reporting agency used for screening will be provided on request. • You may obtain one free copy of your credit report from each nationwide credit reporting agency every 12 months at www.annualcreditreport.com. 5.
